You have to pay for the document preparation, visa, and flight.
It takes up to half a year for all documents to be applied for and issued. Therefore, all documents and information should be in Germany at least half a year before.

If you get sick while working in Germany, you don’t have to worry. You can get treated by a doctor and have your illness certified (the costs for this are paid by your international health insurance).
